Self Referrals to Musculoskeletal (MSK) - Physio  
 

The Southern Health musculoskeletal (MSK) service provides high quality MSK care to the patients living across Hampshire. We offer a patient centred approach to managing MSK problems such as neck, back, shoulder and knee conditions. Our physiotherapists provide a comprehensive assessment and a range of treatments and management plans to support patients to achieve their goals.

About the service

Musculoskeletal physiotherapy teams work with patients who have problems with their muscles, joints and musculoskeletal pains. This includes injuries or conditions affecting your:  

  •  Bones
  •  Muscles
  • Cartilage
  • Tendons
  • Ligaments
  • Joints
  • Nerves

What we do

We provide a service for people with a variety of health problems or conditions. These can typically include: 

  • A recent injury, such as whiplash, knee strain or sprained ankle
  • Surgery, such as knee replacement, ligament reconstruction or shoulder surgery
  • Soft tissue problems, such as after a fall.

 We aim to promote good health in our patients; to this end you may periodically be invited to attend for a health promotion check (blood pressure, height, weight and urine) with one of our Practice Nurses. We can then offer appropriate advice on maintaining/achieving a healthy lifestyle.

This advice can cover: -

Healthy Eating, Weight Reduction, Sensible Drinking, Exercise, Smoking Cessation. Diabetes, Asthma, Hypertension, Coronary Heart Disease, Thyroid and Chronic Obstructive Pulmonary Disease

We have well-established procedures for monitoring patients with Diabetes, Asthma, Hypertension, Coronary Heart Disease, Under-active Thyroid and Chronic Obstructive Pulmonary Diseases. You will be invited for periodic checks to assess control, check your general health and review your medication.

All of the Doctors will provide advice on contraception. Please make an appointment during normal surgery hours to discuss this. Methods available include the contraceptive pill, coil, injectable, barrier and the morning after pill.

Advice on male/female sterilisation is also available. The Doctor will advise each patient as to the most appropriate form of contraception taking into account patient preferences.

Cervical Smears

Cervical smears are offered at 3 and 5 yearly intervals to every woman, dependant on age, between the age of 25 and 65 and can be performed by one of our Practice Nurses or your Doctor.

Once you submit the form you will receive an email response giving you information regarding screening and having a healthy pregnancy. A midwife will contact you when you are approximately 9 weeks pregnant to arrange to complete your booking notes. 

Midwife

A Community Midwife works with the Doctors to provide maternity services. She holds her own Ante-Natal clinics on Wednesday afternoon every other week. She is available to offer help and advice, including pre-conceptual advice, and can be contacted at Blake Maternity Unit (Gosport War Memorial) on 02392 523651.

Minor Surgery and Wart Clinics

Your doctor may be willing to undertake minor surgical procedures after consultation.  Appointments for this can be made after prior consultation - please enquire as to clinic times.

Referrals

doctor2Your doctor may arrange for you to have certain tests: e.g. blood tests, urine tests, x-rays. If you are able you will be asked to have your blood tests at Gosport War Memorial Hospital – between 8.30 am and 2.30 pm every weekday. Blood tests are not conducted on Bank Holidays.

If you are asked to bring a urine specimen for laboratory testing please deliver this to the Surgery before 10.30am.If you are referred for a specialist opinion please let your doctor know immediately if you are started on any new treatment so that your Record can be updated.

Travel Immunisations

If you are planning a trip abroad please complete a travel questionnaire so that we can inform you of any necessary immunisations and malaria prevention. Our Practice Nurses will administer these as directed by your Doctor.

Adverse reactions are very rare but as a Precaution we advise patients to remain in the waiting room for 20 minutes following immunisation. Please allow at least six weeks prior to travelling for these to be completed.

docandchildInfant Development Assessment

Your child’s progress will be monitored by your doctor, midwife or health visitor as appropriate at birth, 8-9 months, 2 years and 3 ½ years.

Counselling

Patient counselling services are available referral being through your GP.
